Florida Metropolitan Transitional Academy (MTA) adheres to a nondiscriminatory policy for all applicants and students. Every student is given equal access to the rights, privileges, programs, and activities offered by the academy.

Student Enrollment Application

To apply, please complete the MTA Student Enrollment Application carefully and submit all required documentation. Applications are reviewed in the order they are received. Once received, our admissions office will contact you regarding assessment or placement steps if applicable.

Registration Form Overview

Applicants will be asked to provide:

• Student Information: Full name, date of birth, gender, ethnicity, and current grade level

• Parent/Guardian Information: Full name, contact numbers, home address, and email address

• School Information:

  • Any siblings interested in attending MTA

  • Type of scholarship (attach affidavit or related forms)

  • Campus and academic year applying for

  • How you heard about MTA

• Required Documents:

  • Proof of birth (birth certificate)

  • Student identification (passport or ID)

  • Parent/guardian identification

  • Medical and health records

  • Proof of insurance

  • Academic transcripts or progress reports (if applicable)

Applications cannot be processed without valid copies of the required documents.

Acknowledgment & Signatures

Both the student and the parent/guardian will need to sign and date the application before submission. This confirms that all information provided is accurate and that they understand the terms of enrollment.

Applicants may also include optional contact details such as email, Facebook ID, or Instagram/Snapchat/Twitter handle for communication updates.
